Objectives

Improving Access to Primary Health Care (Health & Nutrition) Services

Strategies:

  • Deploy Mobile Health Teams (MHTs) to hard-to-reach and underserved areas.
  • Strengthen referral systems between communities, primary, and secondary facilities.
  • Increase availability of essential medicines and medical supplies at facilities.
  • Expand community-based health services through trained CHWs and volunteers.
  • Improve facility infrastructure for quality outpatient and basic diagnostic services.
  • Promote health-seeking behavior through community education and engagement. Screen children 6–59 months and PLWs using MUAC and refer SAM/MAM cases.
  • Support TSFP/OTP sites through trained nutrition counselors.
  • Integrate nutrition screening within MHTs and fixed health facilities.

Enhance maternal, newborn and child health (MNCH).

Strategies:

  • Increase access to antenatal (ANC), postnatal (PNC) and safe delivery services.
  • Strengthen midwifery and nursing through targeted training.
  • Promote early and exclusive breastfeeding and proper newborn care.
  • Improve immunization coverage through outreach and coordination with EPI.
  • Support emergency obstetric and newborn care referral points.
  • Conduct community tracking of pregnant and lactating women for timely care.
  • Increase availability of nutrition screening and supplementation for children and PLW.
  • Raise awareness on danger signs in pregnancy and childhood illnesses at community level.

Controlling Communicable Diseases.

Strategies:

  • Strengthen surveillance and reporting for priority diseases.
  • Support vaccination campaigns and support routine immunization.
  • Promote infection prevention and control (IPC) practices in facilities and communities.
  • Raise community awareness on hand hygiene and sanitation.
  • Support and participate in outbreak investigations and controlling.

Expanding Mental Health and Psychosocial Support (MHPSS).

Strategies:

  • Implement and integrate basic mental health services into primary health care.
  • Train health workers on psychological first aid and common mental health conditions.
  • Establish or support confidential counseling and referral systems.
  • Conduct community awareness sessions to reduce stigma and promote help-seeking.

Reduce Acute Malnutrition among children under 5 and PLWs.

Strategies:

  • Screen children 6–59 months and PLWs using MUAC and refer SAM/MAM cases.
  • Support TSFP/OTP sites through trained nutrition counselors.
  • Integrate nutrition screening within MHTs and fixed health facilities.
  • Follow-up on treated children to prevent relapse.
  • Coordinate with WFP/UNICEF for pipeline and supply management.

infant and Young Child Feeding (IYCF) practices.

Strategies:

  • Conduct IYCF counseling sessions for mothers and caregivers.
  • Promote early initiation and exclusive breastfeeding for 6 months.
  • Promote age-appropriate complementary feeding for 6–24 months.
  • Develop mothers’ support groups at community level.
  • Train health workers and CHWs on IYCF modules and counseling skills.
